Help keeping father at home

My father is 77 and I have him living with me at home. He is aging faster everyday.  I have books under his bed propped up because he can't lay flat because he can't breathe. He has an oxygen concentrator  and a potty chair that was given to us that is unstable on one leg. The shower chair that was given to us has a hole in it so we have to keep a towel over top of it. I need some help to get him on the services so that I can take care of him at home as I have been for the past 4 years. Can someone please help me.
